Are you 60 or older and living with a chronic health condition?
LIFE, Inc. invites you to join our Chronic Disease Self-Management Program (CDSMP) being hosted by Encompass Health Rehabilitation Hospital in Savannah. CDSMP is a PROVEN, six-week interactive workshop designed by Stanford University and led by our trained, supportive staff.
What you'll learn:
Manage symptoms like fatigue, pain, and depression Build confidence to handle daily challenges Improve communication with healthcare providers, family, and friends Discover better breathing, healthy eating, and safe exercise Learn decision-making, problem-solving, and action-planning skillsTopics include:
Appropriate exercise for strength & endurance
Healthy eating for energy and wellness
Safe medication use
Working effectively with your healthcare team
Communicating confidently with family, friends, and providers
Falls prevention & better breathing techniques
