Transition Services

People with disabilities should have the opportunity to choose where and how they live. With the right services and supports, living in the community may be an option for people who are currently living in skilled nursing facilities or other congregate settings. LIFE helps residents explore that option and, when eligible residents choose to make the transition, supports them through the process.

LIFE’s Transition Services also support eligible young people with disabilities as they prepare for life after high school. Through Students for LIFE, young people can explore their goals and choices as they transition from secondary education to post-secondary life.

There are three components to LIFE’s transition services:

  1. Facilitate the transition of individuals with significant disabilities from nursing homes and other institutions to home and community-based residences, with the requisite supports and services.
  2. Provide assistance to individuals with significant disabilities who are at risk of entering institutions so that the individuals may remain in the community.
  3. Facilitate the transition of youth who are individuals with significant disabilities, who were eligible for individualized education programs under section 614(d) of the Individuals with Disabilities Education Act and who have completed their secondary education or otherwise left school, to post-secondary life.
